FEE: $35.00 PER CHILD/per session

INSTRUCTION: MONDAY-FRIDAY 8:00-12:00

INSTRUCTION PICK-UP TIME: 12:00 NOON DAILY
Children
must be picked up no later than 12:30

DAY & WEDNESDAY,
JULY 4TH, INDEPENDENCE DAY HOLIDAY(S).
ATTIRE: CASUAL W/
TENNIS/BASKETBALL/JOGGING TYPE SHOES. NO
SOCCER,
BASEBALL OR SPIKED GOLF SHOES. We recommend you provide your child
with sunscreen, a hat and sunglasses (optional); and a snack & beverage, as
there is no snacks
available at the Wailua Golf Course
EQUIPMENT:
MINIMUM OF A PUTTER, 5,7,9 IRONS, WEDGE, DRIVER AND A GOLF BAG.
IMPORTANT
Parent(s) or Legal
Guardian(s) only of the child must sign the waiver form. If you are not that
person, but are signing the form
for them, you must attach a Power – of – Attorney or notarized document stating you are legally
authorized to sign on behalf of the parent(s) or legal guardian(s). You will be required to
show your driver's license or an identification card to verify your I.D. and signature.
